Post Placement Reports

What does Ukraine require as far as post-placement reports? How many reports must be submitted and what kind? Are they supposed to hear from you a month afterwards, and then get yearly reports? Do the post placements need to be completed by a social worker?

You must register your child within 30 days of returning to the United States. You register your child by filling out a form and attaching a color passport photo of the child. Send this form to the Ukrainian Embassy along with the child's passport. The Embassy will stamp the passport and return it to you.

Once the passport is returned to you, you will need to make a photocopy of the stamped page and fax or mail it to The National Adoption Center.

You will need to submit annual reports to the Ukraine Embassy until your child turns 18. This will keep your child's Ukrainian citizenship up to date.

The annual reports should include a current photo, a brief statement about your child's progress in school, and your child's health. Write this report like you were writing a letter to your best friend.

Ukraine does not require you to have post-placement reports completed by a social worker. Check with your state to make sure that they do not have a law requiring a post -placement report for their files. Even if your state does require you to submit a post-placement report to them, you will still need to send a letter to the Ukrainian Embassy.
